axisymmetric hollow cylinder model with Boundary condition

Hi,

I want to draw a axisymmetric hollow cylinder model which is cut axially along the length. The inner tube radius is 12.5mm, shell side radius is 25mm, with length of 1.5m.

Also how i have to given a boundary conditions to the axisymmetric hollow cylinder model.
